Need a generic form please

I am building a site in Flash 5 and need to create a generic form with drop down combo boxes, check boxes , etc. Any help would be appreciated.

umm... just curious as to what type of help your looking for... The tutorial section has some tutorials on dropdown menu's. as far as check boxes, if you want it to tallie something then you'll need to learn some actionscripting before you even worry about how to create the visual end...
