I havent seen any examples and this rather copmlicated project, becouse you have to combine with one of server side technologies (ex. php .Net etc.)

I doubt that some one will help you at this level. Just start doing it and ask if you'll have particukar problem.

basicly it reqiures to:
-create form in flash (textFields)
-send data to page
