Email Contact Form

I've visited several sites on sending emails from a Flash and using PHP for it or a few of them, XML. I was wondering if anyone could show me a VALID way to send mail from a Flash with a UI Combobox (I've only come across a couple with this), and has more that just 'name', 'email', and 'message' input text boxes. I've tried many ways and countless tutorials but none do seems to work.
