Flash E-mail Form

Does anybody know of a good tutorial on accomplishing this is flash cs3 with actionscript 3? I need to create a form with a name, e-mail, DOB, and 4 different drop down menus. This info then needs to be sent in an e-mail. I have all of the text fields setup and working but am having problems creating the drop-down menus. Any help appreciated!!:)
