Flash form AS to PHP to MySQL Table [Archive]

I have a flash form with action script, which I want to go to process.php then into a MySQL table. I think the PHP and MySQL are ok and there's a problem with the action script. However I am a newby when it comes to .php and mysql so any help would be very appreciated. I have attached the .fla file and process.php but if someone would rather me post the script just ask.
