please help

I've been working on this shout box now for a week and I can't get my head around it.

I've downloaded loads of free shour box's but none seem to work on my site.

This is one that I like the most but when I put it on my machine I cant get it to work.

Something in the read me file says that i need a database and I aint got a clue about them so If there is an easier way to create a shout box please help??????????
