Need Help With Form

Can someone finish this form for me? My site is 95% done, except for this form I've been fighting for a week. It would be much easier if someone can do this for me. I would be forever in your debt.

It may be so hany right now, but then the next time u'll need to make one, u still won't be able to do it. Not to mention everything else with the same principle (sending dynamic content to a server to be processed there). Suffice to say, thats quite a lot. Just keep trying ! :)

Thing is, this is just for my personal site, and lately, I've been very very busy to learn to make the form. My brain right now is on overload with new information, so that's why I'm hoping to find someone who can do me this favor :)
