vote script

Not sure if i am in right place! But am looking for help with script and information on it. I want a vote section on my site where you cannot click "no" and have to vote yes.
Here is an example of what I am looking to create.

I have tried to edit this in flash......but without success. It always seems to play straight thru like a movie......

Please help, any info greatly appreciated
